At Midwest Headache And Spine, our relationship is with you, not your insurance company. Your insurance coverage is a contract between you and your carrier, and not all plans cover every service we provide. We recommend becoming familiar with your policy’s specific benefits and limitations.

If you have questions about coverage or pre-authorization requirements, please contact your insurance provider directly. Our billing team is also happy to help guide you as best we can.

Co-Payments, Co-Insurance & Deductibles

All co-pays, co-insurance, and deductible amounts are due at the time of service.

  • Co-Pay: A fixed amount required by your insurance at each visit, payable prior to being seen.
  • Co-Insurance: The percentage of allowed charges that you’re responsible for after your deductible is met.
  • Deductible: The out-of-pocket amount you must pay before your insurance begins covering eligible expenses.

Insurance Information for New Patients

When scheduling your first visit, please complete your patient information and insurance verification forms (available online or in our office).

Be sure to bring all current insurance cards to your appointment. Providing accurate and up-to-date information helps ensure your claims are processed quickly and correctly.

If you have multiple insurance plans, please let our team know which one is primary. And if your coverage changes, kindly inform us as soon as possible so we can update your records and avoid delays in claim submission.

Depending on your plan, you may be responsible for co-pays, deductibles, or other out-of-pocket costs at the time of service.
